Adult Intake
If you are a new patient, please fill out all of the forms and bring them with you to your appointment along with your insurance card and drivers license. Paperwork can be emailed to you prior to your initial appointment if needed. Please return all paperwork prior to your initial appointment if possible. Thank you.

Insurance participation varies amongst EnSpire Counseling & Wellness providers, so please be sure to verify participation with your insurance prior to your visit.
You should always check with your EnSpire Counseling & Wellness provider to verify plan and product coverage as this is not an exhaustive list, and plans change frequently.
EnSpire Counseling & Wellness will no longer be billing secondary insurance as of 1/1/2022.
All copayments/deductibles are due at the time of service.
COVID-19 Policy
COVID-19 As part of our protocol, our office has heightened its cleaning and disinfecting procedures to maintain a healthy environment for our clients, staff, and clinicians. We are taking precautions and asking anyone who is not feeling well to reschedule their appointment.
In addition to continuing to offer in-office sessions, some of our clinicians are offering telehealth to anyone in the State of GA. Providers will determine if teletherapy is an option per the individual's treatment plan. Please call to determine if teletherapy sessions will be self-pay or filed with insurance.
Please know that the telehealth service should not be used for crisis situations. If you are in crisis, call 911 or 988 immediately or go to your nearest emergency room.
